If you’re planning to travel outside the UK, you may need to be vaccinated against some of the serious diseases found in other parts of the world.

Our practice nurses are available by appointment to offer medical advice regarding travel and vaccinations where appropriate.

Which travel vaccines do I need?

Some countries require proof of vaccination (for example, for polio or yellow fever vaccination), which must be documented on an International Certificate of Vaccination or Prophylaxis (ICVP) before you enter or when you leave a country.

Travel Risk Assessment

We offer a full travel and advice immunisation service. If you are travelling abroad please complete our travel vaccination form in plenty of time (We recommend 6-8 weeks) before your journey.

Non-NHS charges

Some services provided with surgery are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Request a Private Letter

The surgery is frequently asked to provide private letters/reports/ third party letters/To whom it may concern letters. If you request work that is not provided for under the terms of the NHS Primary Care Services contract, we may be able to undertake such work for you, but, as this is a private service, you will incur a charge for completion of your request. Such requests take time as they require the GP to review the patients’ entire medical records to ensure that any information provided is correct.

If your request is accepted – Please note – Private letters may take some time to prepare as requests for these DO NOT take priority over NHS work. We will usually give you a turn around time of 28 days however at certain times this could be exceeded according to demand.

On the rare occasion that a request is deemed to be urgent, then we will do our best to meet your requested deadline – providing this is a period no shorter than 7 working days , however there is no guarantee that we could adhere to this.

We will advise the patient requesting non-NHS work of the cost, the fee is payable in advance. Please note that whilst our GP’s will always attempt to assist patients with their requests there is no requirement that they do so.
Example of private letters/reports include:

  • Letters to the Council re housing
  • Fitness to travel/Holiday cancellations
  • Confirmation of medical conditions/medication etc

Clinicians in the surgery do not counter-sign passport applications also we do not provide letters confirming address/residency, any requests where we have no means of verifying the requested information will be refused. Please note – When a Doctor/Nurse signs a certificate, completes a report etc, they are legally required to only sign what they know to be true.

Test Results

Getting your test results

If your test results show that you need more tests or treatment, we will contact you.

Once a doctor has reviewed your test results, you can view them:

  • in your NHS account (through the NHS website or NHS App)
  • phone or visit the surgery after 4pm

Access to your medical records

Access via the NHS App

You can now access your GP record by registering and logging into your account using the NHS app or NHS website.

From 1st November 2023 patients were given access to their online GP records moving forward (or from the date that their registered with their current GP practice)

If access to ALL online GP records is required, our Online access to full medical records request form would need to be completed and handed in to the surgery alongside appropriate ID as set out within the form.

As a parent, family member or carer, you may be able to access medical records for someone else. We call this having proxy access. We can set this up for you if you are both registered with us. The above form will need to be completed.

Access to Paper Records / Summaries

We would always recommend that patients access there GP records via registering for the NHS app.

If you wish for paper copy/summary of your GP records but are unable to access this via the NHS app, then a Patient Access to Medical Records Request Form would need to be completed and handed in to the surgery alongside the appropriate ID.

Adult ADHD and Autism – self referral process

Patients aged 18 yrs or over can now self refer for ADHD and Autism assessment, there is no requirement to discuss this with a GP prior to using either of the self referral pathways. Please see below Links for more information
